Membership Freeze Request

All memberships:

- No refunds are provided after a freeze has been submitted and fee payment has been made
- Payments for freeze must be made upfront unless advised otherwise by staff
- If freeze is requested for a medical reason, proof must be submitted via email or in-person in the form of medical documentation. In return, this freeze will not affect the member's freeze balance
- If you wish to extend your on-hold form, you must fill and return another 'Membership Freeze Request' form
- Should a freeze fee apply, entry may not be granted until it has been paid. A stop note may be left on your account.

Direct debit memberships:
- Freeze start dates must commence on or after the next direct debit date from the date this form is submitted.
- The freeze cannot start on the same day of form submission.
- Direct debit members have a balance of 4 weeks complimentary freeze and 8 weeks at $2.50 p/week, refreshed annually

Valid upfront memberships:
- Freeze start date must be at least 1-day after this form is submitted
- 6 Months Upfront has a freeze balance of 2 weeks complimentary freeze and 6 weeks at $2.50 p/week, refreshed annually
- 12 Months Upfront has a freeze balance of 4 weeks complimentary freeze and 8 weeks at $2.50 p/week, refreshed annually
