By signing off on the above form, attendees will have the chance to win 1 of 4 One-Month Upfront memberships at DOOLEYS Health + Fitness.

• Each membership prize is valued at $249.00 and includes the joining fee.
• To claim the prize, the winner must:
  • Have no outstanding arrears.
  • Be an active member of DOOLEYS Lidcombe Catholic Club
• If the winner is already a member of DOOLEYS Health + Fitness, their membership will be credited with one month (30 days) worth of membership or have their membership extended by one month
• By entering, participants consent to receive marketing materials and communications from DOOLEYS Health + Fitness and Auburn Ruth Everuss Aquatic Centre Booth
• Each form entry submitted at a unique partner’s booth at the Health + Fitness Festival equals one chance to win.
