I understand that
(1) my membership cancellation requires a minimum notice period of two full fortnightly payments (1 full fortnightly payment if membership started before July 2017) from the date I submit this form
(2) any outstanding fees from my membership period must be paid
(3) my membership cannot be frozen during this notice period
(4) I will have to pay a $200 exit fee should I cancel without fulfilling the contractual payment term of my membership
