Cost: $10 per session/child

I agree and consent to the following:
- For more than two Fit Kids please submit a new booking form.
- Must be a current DOOLEYS Lidcombe Catholic Club member.
- Must be minimum 8 years old maximum 15 years old
- I warrant that my child does not have any medical condition that will prevent or cause further harm by participating in a physical exercise program.
- I understand I must be in the vicinity of DOOLEYS Lidcombe Catholic Club while my child/children are in their session.
- I understand and agree to ensure that my child/children are appropriately dress in fitness gear which includes shirts, pants and shoes. I agree to ensure I provide my child/children with a towel and water bottle for every session they attend.
